Cognitive impairment and low health literacy are common in those with end stage kidney disease, and this impacts how people learn and apply advice about managing their kidney disease.
This project will develop a health literacy focused dietetic outpatient model for people with chronic kidney disease.
The new model of dietary care for people with chronic kidney disease will be rolled out in mid-late 2018 with evaluation of the results expected in early 2019.
